I Stopped Assuming the Manual Was the Machine

Why the gap between procedures and reality is where quality goes to die.

You look at the monthly report and the numbers are red. The first plant in Ohio makes money and the parts are good. The second plant in Texas loses money and the parts are bad. You sit in your office and you look at the binders.

The “Mirror Image” Failure: Identical binders, opposite results.

You sent the binders to Texas and you sent the software to Texas. The procedures are the same and the forms are the same. The managers in Texas are good men and they work hard. They read the words in the binders and they follow the steps on the screens.

But the Texas steel breaks and the Texas tools dull and the Texas customers are angry. You think the problem is the people but the people are following your rules. You think the problem is the machines but the machines are new. You are confused and you are tired.

Instincts vs. Paperwork

The first plant grew over and the people learned the metal. They learned how the heat in July changed the way the coolant flowed. They learned the sound of a bearing before it failed. They did not write these things in the binders because you cannot write the smell of hot oil or the vibration of a floor.

These things are the instincts of the shop and they stay in the shop. When you opened the second plant you exported the paperwork but you left the instincts in Ohio. You gave the new team a map of a city but you did not give them the city. You measured them against a standard and the standard was a ghost.

The Lesson of Adrian K.L.

I was once a playground safety inspector and I was Adrian K.L. and I was wrong about a merry-go-round. I went to a park and I saw the spinning wheel and I saw the gap between the wheel and the ground. The gap was too wide and the code said the gap must be narrow.

I argued with the park manager and I won the argument. I was proud and I felt the power of the code. The manager followed my order and he welded a steel plate to the base. The gap was narrow and the code was satisfied.

A week later a girl ran to the wheel and she tripped and her foot hit the edge of the plate. The plate was sharp and the girl was hurt. I was right about the measurement and I was wrong about the safety.

I stopped assuming the manual was the playground and I started looking at the children.

The Nine-Year Moat

The second plant receives the documents and the team reads the documents. They see a requirement for a receiving inspection and they perform the inspection. They check the boxes and they sign the names. In Ohio the man at the dock has been there for .

He looks at the crate and he sees a smudge of grease in the wrong corner. He knows that grease means the seal is broken and he rejects the crate. He does not need a checklist to tell him about the grease. In Texas the man at the dock is new and he has the checklist.

The checklist does not mention grease smudges. The man accepts the crate and the bad parts enter the line. The process is identical but the result is different. You blame the man in Texas but he did exactly what the paper asked.

We build quality systems because we want to repeat success. We want to take the luck out of the work. But we forget that the documents are the tracks and the instincts are the engine. If you put tracks in the desert but you do not send an engine the train will not move.

Expansion as Photocopying

Most companies treat expansion like a photocopying exercise. They believe that if the instructions are clear the performance will follow. They do not see the thousand small adjustments that happen every day in the original plant.

They do not see the way the quality manager talks to the lead machinist over coffee. They do not see the way the team fixes a problem before it becomes a nonconformance.

I used to believe that a perfect procedure would create a perfect part. I sat in meetings and I argued for more detail in the work instructions. I wanted every movement of the hand to be recorded. I was wrong and I saw the failure of my own logic.

The more detail I added the less the workers thought. They stopped looking at the metal and they started looking at the paper. If the paper was wrong the part was wrong and nobody cared. They were safe because they followed the instructions.

The Drowning Team

The managers at the second plant are drowning in your paperwork. They spend a day filling out forms that nobody reads. They do this because the binder says they must. They lose the time they need to walk the floor and listen to the machines.

They are managed by the ghost of the Ohio plant and the ghost is a hard master. The ghost wants the evidence of conformity but the ghost does not care about the scrap rate. When an auditor comes the Texas plant passes the audit. The folders are full and the signatures are neat.

The auditor is happy and he leaves. Then the scrap bin fills up and the owner screams.

A Living QMS

You need a system that connects the office to the oil. You need a system that takes the burden of the paperwork and gives the time back to the people. A manufacturing QMS should not be a tomb for dead procedures. It should be a tool for living work.

When the system is digital and it is fast it becomes a part of the floor. It does not sit on a shelf and gather dust. It moves with the operator and it catches the errors before the part is finished. In the third plant you will do things differently. You will not just send the binders. You will send the people. You will take the lead machinist from Ohio and you will put him in South Carolina for .

Beyond the Certificate

He will not teach the manual. He will teach the metal. He will show the new team how to feel the heat and how to hear the vibration. He will tell them about the grease smudges. The new team will learn the instincts and then they will use the system. The system will hold the instincts and the instincts will drive the system.

The steel does not care about your ISO 9001 certificate. The steel does not care about your IATF 16949 goals. The steel only cares about the heat and the pressure and the tool. If you give the steel the wrong pressure it will break. It does not matter if your paperwork is perfect. The part will be scrap.

You must build a bridge between the document and the machine. You must admit that the manual is an approximation of the truth and not the truth itself.

The 0.002 Gram Defeat

I won an argument once about the calibration of a scale. I insisted the scale was out of tolerance by 0.002 grams. I was right by the book and the scale was pulled from the line.

Lost in one afternoon

The price of valuing the document over the purpose.

The production stopped and the company lost eleven thousand dollars that afternoon. Later I found out that the parts we were weighing were large iron castings. The tolerance of the parts was ten grams. My 0.002 gram victory was a defeat for the company.

I was right about the rule and I was wrong about the reality. I see this same mistake in every plant that tries to scale by the book. They value the precision of the document over the purpose of the business.

Exporting the Light

Your second plant is not failing because the people are lazy. It is failing because you gave them the artifacts of your success and you kept the source of your success for yourself. You exported the shadow and you kept the light.

To fix the plant you must stop looking at the binders and start looking at the floor. You must find a way to capture the unwritten rules and put them into a live system. The rhythm of a good plant is a beautiful thing. It sounds like a drum and it smells like ozone. You can feel it in the soles of your boots when you walk through the door. You cannot put that rhythm in a three-ring binder.

You can only create the conditions for the rhythm to start. You do that by giving the people a system that works for them and not against them. You give them a system that proves the quality and does not just claim the quality.

Then you step back and you let the machines run. You watch the trucks leave the dock and you look at the report.

BLACK NUMBERS

The numbers are black and the parts are good and you are no longer confused. You are a builder of things and not a keeper of papers. That is a good way to be.
